Lean tissue index as a determinant of achievable substitution volume in hemodiafiltration

In this Research in Brief, Yan Zhang, Senior Expert in Real-World Data Analysis at the Renal Research Institute (RRI), discusses whether body composition, specifically lean tissue versus fat tissue, better explains achievable substitution volume in hemodiafiltration (HDF). 

In this study, Zhang and colleagues analyzed the treatment data of nearly 40,000 patients treated with HDF across Fresenius Medical Care clinics in the Europe, Middle East, and Africa (EMEA) region. The results showed that lean tissue mass, not fat mass, was the key determinant of achievable substitution volume. Patients with high fat but low lean tissue index consistently achieved lower substitution volumes, while those with higher lean tissue index achieved higher substitution volumes regardless of fat levels. This suggests that body composition-based parameters, especially lean tissue index, may provide a more accurate way to individualize and interpret HDF adequacy. 

Ultimately, by shifting focus from body size to body composition, physicians can better optimize the convective dose delivered in HDF.
